VA MEDICAL RECORDS
If you’ve been treated by the VA get a copy of your VA medical records by completing VA Form 10-5345a and submit it by mail, fax or in person to your VA health facility’s medical records office. This office is also called a Release of Information Office.

MILITARY MEDICAL RECORDS
Health records cover the outpatient, dental and mental health treatment that former members received while in military service. Health records include induction and separation physical examinations, as well as routine medical care (doctor/dental visits, lab tests, etc.) when the patient was not admitted to a hospital.  Where these records are kept depends on your service and when you ended your service.

CIVILIAN MEDICAL RECORDS
In order to obtain a copy of your medical records from your civilian doctors you have to contact their respective offices and utilize their procedures. Depending on the organization those records may already be available online through an account they established for you. Check with your doctor’s office to find out the best way to get access to your records.
