DD-214

A DD-214 is a Report of Separation which is generally issued when a service member performs active duty or at least 90 consecutive days of active-duty training. The Report of Separation contains information normally needed to verify military service for benefits, retirement, employment and membership in veterans’ organizations.

When you submit a disability claim it must include a DD-214.

If you don’t have your DD-214 you can request copy. REQUEST DD-214

Spouse and Family Members:
Spouse and family members can also request a copy of the veteran’s DD-214. You can request it online or download and the SF-180 form, complete it and mail it in.
